2016 3rd Annual Living Smart Workshops” Health Expo & Food Drive


                                      G3475 Refugeereetings Vendors:

We would like to extend an opportunity to you to participate in 3rd Annual Living Smart Workshops Health Expo & Food Drive on July, 30th 2016 We are looking forward to having many of you back and many new vendors. We will be on the grounds of New Birth Christian Ministries 3475 Refugee Road Columbus, OH 43232. The event will begin at 10:00am until: 6:00pm. Vendor setup time will
begin at 7:00am. Tear down will begin immediately after 6:00pm.There is approximately 100 vendor spaces available again this year. Vendors will be responsible for their own tent, chairs table,
and table coverings. A 20x20 space will be provided and designated once you arrive for setup as well as volunteers to help get you checked in and unload your merchandise to your space.
Please return your vendor application, cashier check/ money order and signed release forms as soon as possible to ensure your 2016 vendor space. All vendor applications and vendor fees must be received by June 30th, 2016. Spaces will be filled on a first come basis.
  1. Fill out the application thoroughly. Vendor Spaces will be honor on a “First Come” Basis.
  2. Read the Rental Agreement and Release of Liability Carefully.
  3. Fill out and include a current email address for Vendor Space Confirmation.
  4. Enclose your Cashier Check/ Money Order made out to Professionals Golfers Making a Difference.
  5. Spaces are available until June 30th, 2016
  6. Mail the following: Application and Cashier Check / Money Order ASAP. to
Golf Professionals Making a Difference, 789 Science Blvd. Gahanna, Ohio 43230

Vendor Space Confirmation emails/calls will be sent by Mid-June

We will do our best to accommodate any special requests but there are no guarantees. Make sure you read the rental agreement and sign the part of the application that states that you agree to the rules.

 2016 3rd Annual Living Smart Workshops” Health Expo & Food Drive

                                         2016 Vendor Rules/ Regulations Agreement


  1. The event starts at 10:00am and ends at 6:00pm on July 30th ,2016, 2. Set up is Saturday Morning July 30th 7:00am-9:30am.
  2. Vendors setting up are required to sign a release of liability, which is on the application. Make sure you read the release on-line before you sign on the application.
  3. Spaces are approximately 10x10 and 20x20 for food vendors. YOU MUST KEEP YOUR DISPLAY WITHIN YOUR SPACE. If this is not an efficient amount of space, please rent an additional one.
  4. Vendors must maintain a professional manner in selling and displaying their goods. All vendors are limited to one sign and this must be kept within their designated area. The vendor must provide all necessary display equipment. The vendor must agree to remove any part of their display that’s deemed inappropriate to Living Smart Workshops” Health Expo & Food Drive.
  5. Vendors are solely responsible for all financial transactions connected with their merchandise
  6. The sale of commercially produced goods or original products IS ONLY PERMITTED! No knock off or bootleg items are permitted and authorized to be sold on the premise. In the event of a dispute, the decision of Living Smart Workshops” Health Expo & Food Drive will be final and no refunds will be made. You will not be invited back to participate in next years event.
  7. All Vendors are solely responsible for obtaining the proper licensure; Food vendors must have all health & food permit and/or license & insurance.
  8. Due to Fire Regulations there are to be no open flames, lighted candles, or smoking permitted . Only a limited number of sponsors will have electricity Vendors who need electricity must provide their own and use a regulated generator, only utility grade extension cords may be used. No household extension cords will be permitted. Violations will result and no refunds will jeopardize their opportunity to participate in the event next year.
  9. No Vendor is allowed to sell beverages at this event .Pepsi Products will be available at this event only.
  10. No pets are permitted at any time.
  11. Living Smart Workshops” Health Expo & Food Drive, and its affiliated organizations, and their members are not responsible for any damages, loss or theft of merchandise or display equipment.
  12. Living Smart Workshops” Health Expo & Food Drive agree to publicize this event through various types of media in and around the greater Central Ohio Area.
  13. A vendor fee of $50.00 per space non-profit/ informational/literature , $100.00 per space selling merchandise and $200.00 per space 20x20 for food vendors is due by June 30th, 2016
  14. Applicants need to return the following by June 30th, 2016
    1. Application with signatures.
    2. Cashier Checks/Money Orders for the Vendor Space Selected.

Vendor space confirmation email will be sent out by mid-June. There will be no refunds for cancellations.

We feel these regulations are necessary to insure the success of the event for all of our participants. Please keep this list and refer to it before the event.
